The Georgia State Board of Pharmacy has adopted Board Policy #9, establishing guidelines for initial recognition and periodic review of Board-approved treatment providers and/or evaluators for pharmacists and pharmacy technicians diagnosed with, or board-ordered to undergo evaluation for, substance use disorders or mental health disorders impairing their ability to safely practice pharmacy or work as a pharmacy technician. 

Accordingly, all currently Board-approved treatment providers must submit an application for Board approval. This application may be found on the Board’s website. On June 30, 2026 the Board-approved treatment providers list will be updated, and any currently approved provider who has not submitted an application will be removed. This requirement applied to providers located in Georgia as well as those operating in other states or U.S. jurisdictions. 

Providers operating at more than one location must submit a separate request for each location.
